Healthcare Support
Services are non-emergency. They do not replace primary care, home health or emergency services. No diagnosis, prescribing or invasive medical procedures are rendered. Services are provided under the RN Scope of Practice and adhere to current state regulations, as is following evidence-based nursing standards. Services rendered will uphold legal, ethical and professional nursing standards.
Clinical Assessment & Triage
- Perform comprehensive health assessments (in-person, telephonic, or virtual)
- Utilize RN clinical judgment to triage symptoms and determine appropriate levels of care
- Identify urgent, emergent, or preventive care needs
Care Coordination & Navigation
- Coordinate appointments with physicians, specialists, and diagnostic services
- Navigate patients through complex healthcare systems
- Facilitate referrals, authorizations, and follow-up care
Patient Advocacy & Support
- Act as a patient advocate to ensure safe, appropriate, and timely care
- Communicate patient needs, preferences, and concerns to healthcare providers
- Support informed decision-making using evidence-based nursing practice
Health Education and Coaching
- Provide individualized patient education based on RN scope of practice
- Educate patients on diagnoses, medications, treatment plans, and preventive care
- Promote wellness, disease prevention, and lifestyle modifications
Medication Management (RN Oversight)
- Review medication regimens for safety, interactions, and adherence
- Educate patients on proper medication use and side effects
- Collaborate with prescribers regarding medication concerns
Post-Hospital and Transitional Care
- Provide RN-led follow-up after hospital or procedural discharge
- Identify potential complications early and intervene appropriately
- Reduce readmissions through proactive nursing support
9. Collaboration with Healthcare Teams
- Work closely with physicians, therapists, case managers, and social services
- Participate in interdisciplinary care planning
- Serve as a clinical liaison for patients and families

During this time, I gained valuable experience as a med surgical nurse before transitioning into critical care. Just as I advanced into management, COVID hit, after which prompted my choice to relocate back to Florida.
